Build one page data entry form and a reporting section

I would like to build a simple website: 1. User page to login with roles for specific users. 2. One page for data entry using a form (the form is fixed). 3. One page to generate reports out of the entered data. The most important thing about the form is the data validation roles. The website should be RTL. I am not looking for a fancy website. No companies, no groups, i need an individual to do it. More details will be provided upon selection. I have a very limited budget.
