213611 landing page and simple form

I have two pages that need to be developed 1 - simple page with text and one box graphic (sign here type of thing). the second page is a form with just 3 fields Name, Email, Phone#. this form needs to ensure that all three fields have their data. and then just send this info via email to my email. I have a simple template for this project from their existing site. So these pages match up with the site. they will be directing traffic to these 2 pages. I will send you url of the site via PM.
