PHP Bootstrap mysql javascript website

I need help with a php mysql bootstrap 3+ webpage skeleton with a hierarchy login system admin/management/worker/client/guest so 5 different groups/roles. Also put a ewcid shopping store widget inside. the login for client. -Login with (email/username) if admin/management license # + password for client worker ID + password for worker guest login -admin page shows all users and can delete them on a table. Also look at all operations from management/client/worker stuff. Can edit work orders etc. -management page - area to assign work order to each worker, and see the forms for each client job. - worker page - see assigned work order # from manager. Click order # and form pops up with with clients information/picture of worker and area for video. (already have client data in mysql database) roughly 1300 records most permanent. Form must contain all client information name,license, dba, phone ,address, city zip etc. plus an area to enter data such as weight, also button to timestamp when job is started and button to timestamp when job is done. This info would go into tables for that specific work order number. So If management or client wanted to pull up a form then they could see that information for that work #. -clients pages with specific work order # forms they have. only view no edit. + shop center on tab for clients only. - guest are routed to home page about/team/service/etc normal webpage facts set up. I have something set up now but it's only two tier login admin/customer. I am hosting on godaddy, cpanel. If you need more info please let me know. Dont have much to offer but I need help. thank you!
