wordpress webpage form create

Hello freelancers, I have a PHP web form that is a multi-step form that I need broken up into separate webpages. The web form that I need broken up is located here:[login to view URL] This web form is currently one webpage ===order.php. I need this form broken up into four separate pages. I need a number search page, number forwarding page, account sign-up page, and confirmation page. I am currently using this one [login to view URL] page as an I-frame in my WordPress site. I can no longer use it as an I-frame due to web tracking and gola setting. I have to create the web form within Wordpress. This is why i need them broken up. I need a freelancer to break up each section of this web form and create independent pages within word press by entering in the code directly into the content management system. Any PHP variables must move from one page to another just as they do on the [login to view URL] page. I do not need the outside border of the page. But I will need the images and colors that are included inside of the border. Please respond to this bitd with "hello dingo" so that I will know you have read correctly.
