Subscribe to our newsletter Form with unsubscribe feature

I need to add an option to subscribe to our newsletter on our website. Important: The DIV has to match the design of our website. (so developer should have design skills) Subscribe, unsubscribe features are required. Submitting of form and form validation should be ajax. When an email is added to the database; status=1 , when email is unsubscribed status is updated to null, if a user wants to subscribe again, then status will be updated from null to 1; table structure is below.
