Design a transcript

Need a graphic designer to make a PSD of a college transcript. While I can get the real one from my university, time is of the essence and the requester just needs to see anything while we await for the official one to be sent. I will supply the various graphics needed and more detail to the winning bidder. Should be very similar to the one I have attached. There will be slightly more detail. Please attach samples (if available) of work and indicate how long it will take to produce.
