Create a simple multiple choice test for our phone staff manual

We basically want to create a simple paper based multiple choice test around our 'Phone Staff Manual' in our company. Basically, we want to be able to offer a quiz to ensure the employees read the customer service manual properly. It should have trick answers to make the test harder to pass.
