Create a Replica of a PDF Report in MS Access / VBA

In this MS Access / VBA project, you will create a visual replica of a moderately complex, four page medical form. Your project will include a light-gray data entry form with data entry controls (text boxes, check-boxes, etc) that looks somewhat similar to the PDF report in order and control placement. The form(s) will have a print button that will open an Access report that looks almost exactly the same as the paper/PDF form, using data from your data-entry form. The four-page form has a few hundred fields on it. The last page only has a free-form comments field. Your project will be integrated into a larger system, so there is no requirement for anything except the data table(s), entry form(s) and final report object. Design constraints: - Your project must be completed in a .mdb file format (.accdb will be rejected) - Any coding and object naming must use Leszynski's LDF/hungarian naming conventions with camel-case (if you don't know this, please reference [login to view URL]) - Your table(s) will include a foreign key for a case_number (integer) - There is a minimal need for normalization as this form will be tagged onto a much bigger case file. -Table keys must have an autonumber key as the first field in the table. Your tables will be converted to SQL Server -Database tables and fields must not use reserved words for Access or SQL Server (ie. don't call a field "Date", or "Field" etc) -Field names must not use spaces -You can complete the work at your site and present with Access tables, or on SQL Server linked tables in MS Access -You will provide a TSQL script to create any tables/views that you use in the report. -Firm bids only please, no negotiation after award.
