Make changes to my existing order form

I sell subscriptions in a custom online store. Here is the test site. [login to view URL] I sell three types of subscriptions. Each subscription has it's own set of options. We use Stripe as a payment processor to bill the customer and manage recurring billing. I need the following changes made to the order form. 1. There are additional questions I ask after the product is chosen. Different products get different questions. Instead of using Jquery for the entire list of questions for every product, I want to move to a multiple page format where each decision loads a new page. I want to keep the URL looking clean so variables can either be passed as session variables and cleared on prior pages or you can use nested folders to know where we are and what questions were answered for each product. 2. Currently there are three initial questions. These are my products. The responsive site puts each choice on top of each other when the browser is narrow. The site makes a three column list when the browser is wider. I would like to change this so that instead of three columns, there are two columns with images on the left and text on the right. For narrow views, leave as is with text under the images. 3. I want to add a fourth question/product and have a uniform method to add a fifth/sixth/seventh down the road. Use the same questions as Yarnbox Luxe. Use a placeholder image for the new product...Or just link to an image that doesn't exist for now. The price of the new product will be $54.95 4. I want to eliminate three and six month subscriptions for Yarnbox Classic. We only are doing monthly and bimonthly. 5. Shipping - I charge a different amount for shipping based on country. Currently we had a multiple plans in Stripe for each subscription, based on country. We now want to have two separate billing methods for US customers. Basic and Expedited. No subscriptions will offer free shipping anymore. I want to move to one plan for each subscription and use a 2nd line item in the future subscription invoice to bill for shipping. 6. Currently the logic is faulty for billing current customers. Regardless of what credit card the current customer uses in the form, Stripe bills the card that they currently have on file. For current customers, I want an option to use the current card on file OR to use a new card. I know how to code. I build 95% of the site myself and I know how long this should take to do. If you would not consider yourself an expert with experience then do not bid on this project. I want someone I can trust will do the job correctly with little help from me.
