HTML/PHP Job Application Form

We are needing someone to create a php/html/java(if needed) driver job application form. This will be separated into nine different php/html pages. We will provide an example paper application with the required input form entries. This will be hosted on a Linux/PHP/Apache (Debian) server. Basically it is just a Truck Driver Job application form. Drivers will be able to enter their data on each page. We want to have error checking on each page after hitting the submit button. If something required is missing, then display an error with what is missing. All input values should be sticky – For example: <input type="text" name="first_name" value="<?php if(isset($_POST['first_name'])) echo $_POST['first_name'];?>" /> After each page is submitted, we want all entries to be entered into SESSION data so we can use it at the end after the entire application is completed. Some of the entries might require more lines of the same data, so we want a “Add more” button or something, should the drivers have to enter more data, For example, the employment record (history) on page 3 should have of the same entry blocks of data for each previous job, but if the driver had more than five previous jobs, they should be able to hit a button to add another previous employer block of data to enter. So basically they would be entering six previous jobs. Prior to hitting the submit button, the last item on each page should be a checkbox stating something like “By checking this box, I certify that all is correct to the best of my knowledge”. This box might be page1_checkbox, page2_checkbox in the SESSION data. Four of the nine pages are just text for the driver to read and certify that they read the page (with the checkbox). The other four pages are basic job application data. We will send you a copy of the paper application so you can format each page as close as possible to the paper application. The last page (page 9) is the signature page. We wish use jSignature (Williwsystems – open source) so the driver can sign the page with a mouse or tablet, and we can capture the signature in our data base. We can send you the text in a doc format for the text on some of the pages.
