Form management development of ezUPLOAD

My client is a modelling agency. Models apply to join the agency and provide all their information, including uploading photos, through a form on the client's website. My client currently uses a program called ezUPLOAD 2.2.5 to set up the form and manage the applications and the data is stored in a MySQL database. Whilst ezUPDATE provides good information, the client needs additional information and functionality from the database. Key information about each applicant should be displayed in a table with the ability to select models for audition (Yes/No). The table should include thumbnails of the uploaded images not the images themselves. Those who are judged No should be moved to a separate page(with the opportunity to change then back to a Yes). Those models who are selected with a Yes need to be sent an email from a template (which may need to be revised from time to time) which will have the ability to have a date and time for an interview to be added. A complete list of fields will be sent to the sucessfull bidder. Special consideration will be given to anyone who has experience of using ezUPLOAD
