Fix my Form - Site hosted in AWS (EC2) and database created in RDS

I am using AWS to host my site: [login to view URL]
and I can't get to make the form work when I press the SUBMIT button in the contact form

The database was created in RDS and the site is hosted in EC2, the issue seems to be that I am not able to connect them.
I need the form fixed ASAP. If you can do it send over how long it will take you to fix it, and Fixed Rate for the project.
We will not accept PRICE INCREASES at any point in the project.
