Episerver form page

Build a form page in Episerver with email posting functionality. Depending on choices made by the visitor two different paths will be presented. First step is to choose between indoor och outside, second choice will if indoor is choosen be 1st or 2nd floor and if outside is choosen go direct to name, phone and mail address fields and then a send button. This form should be placed on a EPIserver. The result of the form should both be saved on the EPIserver and also be sent to a mail address. ONLY persons with Episerver experience will be hired for this project.
