php form with calculations

I need to have a form designed that will take a set of kitchen cabinet dimensions, perform a series of calculations on them and return a result to the customer and save those same results in a db. I want this form to be listed in the Zen Cart "shop pages" and linkable from my product page files. The calculations are rather simple and consist mostly of logical calculations. I am currently performing these on an excel spreadsheet (from submitted dimensions), but I would prefer that customers get instant answers via online submission.

The interface for this form will be on a ZenCart platform and should not require any modification to the platform.

I will provide artwork for the form and the results needed from the dimensions.

This work must be performed quickly and will require the re-writes necessary to achieve a perfect solution. An understanding of kitchen cabinets is essential.

Here is a breakdown of the major components of the job:

1) Collect 6-8 dimension measurements

2) Perform algebraic calculations to arrive at results

3) Format results within text paragraph(s) incorporating the results with product recommendations based on a "lookup" of our product db

4) Assign a unique id to this submission/result and save it in a new db

5) Allow for admin viewing of these db entries in a viewable inquiry screen
