We currently have a wordpress site. We have built forms using a plugin called formidable pro ([url removed, login to view]) which is a PHP form creator for wordpress. The plugin creates form where the data from submitted forms are saved to MYSQL database. We require the saved data to be merged into an existing document / template and emailed as an attachment. This must be achieved by creation of a wordpress plugin so that future forms created can be easily selected to be merged to a word document to be emailed.
So far the forms are already created. The data simply needs to be pulled from the database and merged into the document and emailed as an attachment to two email address: (1) an email address provided by person completing form; (2) webmaster/admin's fixed email address.
We will provide the Word docs with placeholders for where data should be merged to.
