Photoshop design needs to be sliced and properly coded into live site

I have a site design in photoshop that I need correctly sliced and coded for a fully functional web site. No database needed - same header/footer for all pages with simple text rollovers for hover. Also will need a php email form coded to be functional with checking for input into the text fields, radio buttons for choices all sent to the site visitors email as well as the site owners email through php with a confirmation the email was sent (alert pop up box OK) clear form and return visitor to home page - all design is complete, just need someone to slice and code. Photoshop file will be provided - 10 pages in total, most are very simple text for the content with header/footer/background staying the same for all pages.
