Script will need to do the following and be easily integrated into a website I am currently developing.
The script will need:
1. backend- login page with username and password for an administrator to add, modify or delete records in the database.
2. front end - one input form field on a webpage to allow a user to enter a code plus submit button.
BACKEND DETAILS
an admin page to allow an adminstrator to login via username / password. Once logged in they will have 3 options... Enter a new item, delete an existing item or modify an existing item.
Whe creating a new item the admin needs to see 2 fields... the first is an input box to enter a 10-16 digit code, the second is a drop down box with 3 options- each option is an output message.

"positive 2"
"negative"
(only one may be selected) this value is stored with its corresponding ID code.
FRONT END USER INPUT DETAILS
Upon entry the code will either be found in the database and take the user to a results page to show the results of the corresponding code. Or they will receive a message stating "your results have not been posted yet"
If the code is found in the database the user is taken to a results page to see the results corresponding to their code. The results page will display the ID number then the result in this fashion (bold text are the values that need to be pulled from database):
Kit ID #: **XXXXXXXX
**Results: **result message**
