Major Components:
**Form Creation:** The ability to create a form using existing fields or new fields, exporting that form to .html, for later manual including in web-pages.
**Data-Set Gathering:** The ability to centrally gather all data-sets the different forms create into one database whose header-row consists of all field-names ever created.
**Data-Set Distribution:** The ability to press a button to email a certain data-set to one of several pre-entered clients. Naturally, there should be an ability to enter and modify these clients.
**Data-Set Tracking:** The ability to see client by client, day by day, what data-sets have gone where, with a re-settable total number of data-sets sent.
Detailed descriptions of components:
**Form Creation:** Basically, we want to be able to create a form with submit function. The form should submit to the database in the system. The unique form should be created using pre-existing and/or new field-types. For example, almost all our forms will have “name?? and “email address?? as fields, but only one will have “description of accident??. Every time a new field-type is added, that field should be added to the database as a new column. We imagine this part of the program functioning something like this:
1. New Form
2. Insert field
a. Choose field /create new
3. Repeat until done
4. Save (Force Naming of form)
5. Export to file
After the file is exported, we would open it in notepad, copy the code, and paste it into the table on the page where we wanted that form.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
