FrontPage2002 & MySQL (62974)

We need for our Web (developed in FrontPage 2002) a Price selection box and one Customer registration form, both Price selection box and Customer registration form must be linked to our MySQL database. Price selection box ------------------- We are a publisher of commercial Time Management software and offering our software in two versions; Professional and Standard Edition. The new pricing selection box will replace our current price list. The price box will contain the following fields: ---------------------------------------------------------------------------- Software version: SSSSSSSS (Switch option: Professional or Standard) ---------------------------------------------------------------------------- How many Employees in your company will use Atlas: 50 (Quantity) One time List Price: 123.45 Annual Maintenance and support Fee: 123.45 Annual Lease price: 123.45 Annual Maintenance and support Fee: Free Monthly Lease price: 123.45 Monthly Maintenance and support Fee: Free ---------------------------------------------------------------------------- The visitors will only have to select the required software Edition (default will be the Professional Edition) and enter the Quantity. Pricing details will be obtained from the MySql database. Pricing database record ----------------------- The key to the pricing database is made-up as follows: AAAEEQQQ , • AAA = Application Code, will be a constant value “TMS?? • EE = Software Edition, PE (professional) or SE (standard) • QQQ = Quantity of Employees entered (always 3 positions, 001, 015, 125, etc.) For pricing records not found in the database display the message: Please contact our Sales Department: oursalesemail.address. Thank you. Customer registration form -------------------------- When the customer pushes the Order Now button, the customer registration page will appear. This page is not directly accessible, only when called by the Order Now button. The customer registration form must contain all usual company and user name, password and address fields, state and country, etc. We will give you the list of field required by registration. A unique temporary Customer code must be assigned by the program. Customer number should be stored in a Cookie. All details must be stored in a MySQL Customer database.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
