fix line breaks in Fortran 77 code

I want to use the code at [

[url removed, login to view]][1] but the line breaks are missing. Please add line breaks to all the files so that they are Fortran 77 code that can be compiled with a Fortran 77 compiler such as g77.

## Deliverables

Compilable Fortran source files, compressed to a zip file.
