Web Page

I am looking for someone to design a web page for me. I have an existing web site that I am happy with for now. I own a mobile car detailing business and on my website I currently have a simple prices page that uses an image of my price sheet for customers to decide which detail they want for their car. What i'm looking to do is have a page created that would allow customers to use check boxes and drop down menus to select the services they want to have done. Based on their selections the options down below would change and eventually recommend one of my detail packages for them as well as give the price and description of it. For example there could be a drop down box that would let them choose if they wanted an interior or exterior wash or both. Based on that it would pull up a form or a new web page that would give options. For example if they choose "Interior Only" it would pull up a new form or web page that would give them options with check boxes for example: "Carpet Shampoo" "Seat Shampoo" etc... Then based on what selections they make it would match those to one of my detailing packages and return another web page that would show that detail package and its description and price. I don't want any design work done to the page itself, I would want the page to look just like the page I have now just with the body different.
