279029 Easy Javascript Fix & HTML

I need someone to fix the javascript on this page: [login to view URL] At the top of the page you enter your name and clickbank ID and hit submit. That is supposed to populate the html code for the banners lower in the page (replacing {aff_link} but it isn't inserting for all the banners. I also need 25 more banners to be added to the page (using the format show with the other banner - banner image followed by html code) - I will provide you the URL for the banner images. Note:In order to be considered for this job you must verify that you have read the all the project details. To do so please include lion37 in you reply/bid/PMB. Remove  which shows up at the top of the page. Finally please remove the yellow/peach colored border around the page.
