486707 Add simple data form to a Wordpress page

This form will go to the PapaJoe-dot-org website that is already live. I simply need a form on a page that has the fields I'll paste below, and aside from populating an easily queried database, the each form submission should create a simple post to go into our Choir Applicant category and contain all form data provided (while being visible only to admins). Simple stuff for a pro. I want to be able to change the fields in the future, so if it's easier/cheaper for you to activate and test the plugins and database and let me enter the fields that show on the page, that would work too, but here are the fields we'll want: Name: Gender: Age: Race: Voice Type (Soprano, Alto, Tenor, Unsure): Actively singing?
