Write/Modify a SQL report for a Volusion store

need a sql query written for a Volusion store (which means not all SQL functions work)
results needs to look like the attached spreadsheet and I have provided a sample query which has the columns and the tables needed to get the data.
what I need is it to be done like a pivot, but pivot doesn't seem to work in this particular SQL instance (Volusion), so an aggregate CASE WHEN would be ideal, I just don't know the syntax for that.
so the summary of the report is, need a report of sales total by customer, by month (months in columns) to be able to compare one month to another.
