PHP Video/Audio Converter to HTML5

I need a php page that I can send a request string to.
The request string will have the file path of the file to convert.
The request string will also have the file path of the folder to save to.
The request string will also have the file name to save the files as.
The request string will also have the length limit in seconds to save the file.
That page will create the HTML5 video or audio files then save them to the folder.
Input: All common type of video and audio file formats.
Video Output: MP4 WebM Ogv
Audio Output: MP3 Wav Ogg
This could be a Page Function or Class. Whatever you think is best?
Server: I have a dedicated Apache Web Server, MySQL, PHP with FFMpeg Support.
