Custom Form Submission Website Programming Project

The system consists of two main interfaces, one for users and one for the admin. The user interface will be based on a document file questionnaire that an employee would originally fill out by hand. I will supply this document and further details of how the form works if I feel you are a good fit for the project.
User Interface:
Mobile-friendly website.
Login page with username, password, and "remember me" option.
Form page with signature box and simple diagrams. Data is saved continuously, and abandoned reports trigger email notifications. Hidden fields record initial form date, user name, IP address, browser, device, and current location.
Upon submission, data is saved in the database and emailed to multiple manager addresses. Users receive a confirmation code.
Admin Interface:
Ability to manage users: add, deactivate, or modify.
Admin settings for notification email setup.
Summary listing page to view all records in the database, with options to:
View full details of form submissions.
Add comments and attach files to submissions.
Mark submissions as incomplete.
Sort by submission date.
Search with parameters.
Filter incomplete/complete submissions.
Print form submissions.
Email copies of form submissions.
Requirements:
YOU and only you are the programmer and will communicate and work with me on a daily basis.
Must speak English well so we can speak on phone call through freelancer when required.
1 WEEK DEADLINE, NO EXCEPTIONS. I recently had an issue and lost a month. I will be available everyday to work side by side with you to ensure completion, bug free.
