jquery form validation and automatic results changing criteria

I have a form with 5 drop down fields. Let's say they are numbered from 1 to 5. The first first drop down selection changed the fields in the second drop down field due to availability. The results are updated immediately in the form. The same sequence of events from fields 2 through 5.

I will provide a dummy page, and the mysql tables that are involved. I can also provide an example of a similiar functional website.

This will most likely take a few hours from a professional. Therefore I'll place a milestone for the full amount and release it when the job is complete.
