Graphical Design for PHP Script

Hello,

I have a very simple php script (about 30-40 pages in total - most of them VERY simple) that needs a nice graphic design so that it will look very professional. I have uploaded a picture of the 3 most complicated screens. The rest of the screens are either almost identical or much simpler than the ones shown.

The successful bidder will design a nice looking template, and once the template has been approved by myself, will integrate the template into the existing php script.

I would prefer to have several mock ups of designs to choose from before deciding on which one to go with, although I am not insistent on that if you are confident that you can provide a very professional looking layout on the first try. However I will not accept amateur looking designs.
