Urgent-Need PDF report generated from PHP app

The web-based application in question is a four part process whereby the user enters their contact information, then enters data, then submits. The application posts the data to a DB and generates a report by email which is sent to the user. At present the email is html format and replicates the tables from the DB. However, it is insufficient.

I need a script installed that will generate a PDF version of this email. The PDF needs to include the client contact information, a brief introduction to the user, and then the data from the application.

I understand there are open source PDF applications that can be installed on my linux server to accomplish this.

I need this done as soon as possible-today if preferable.
