PHP Form

Hello,

My friend has got such form done for $30 bucks. So that's how much I could pay for this task, up to $44.

It's a 2 step PHP contact form.

The user enters details, selects from drop down menu, and according to the selection, after a form submission he's sent to one or another page (redirected) where all this info is pre-filled already, plus he has to put additional info and then send the data to my email.

I’d like it to be simple so I could manipulate this myself. Put a redirection to one or another page according to the selected field from a drop down menu. I'm not good at PHP, but I could duplicate some code lines and create additional fields and stuff.

So I'd also need a little help of Yours to explain me how to edit the form later, if it's ok with You.

The should also be a PHP validation for fields that I'd choose to. I'd like to have the ability to make some of the fields required and some not (I'd need to ask a few questions in this case).

The form, if filled wrong, should not redirect me to another page. It should refresh with the missing data fields at the top, and red bolded fields next to each field in the form below in the same page. Like here:

I'd also like this form to be secure. So some spammers wouldn't be able to use it to send some spam...

"It’s important that the Form Mail Script is called by a form or link from your website. For safety reasons the reference site is checked and compared with the script specifications. If the information agree the script is executed."

Maybe something like that, I don't know. I'd just wouldn't like some spammers to use my form in some way.

One other thing...

Would it be possible to create a javascript show/hide form fundction? In case a person selects something from a drop down, the other form below appears or disappears. I think it's done with CSS and Javascript.

That's basically it, I could pay up to $44 so far...
