I need two forms created in PHP, one that will email the results of the form in a CSV file to an email address. The other one which will just simply email the text of the form to an email address.
I have attached a document with screenshots of how I want it to look and function (doesn't have to follow the current color scheme, I will add a CSS once the project is done).
No database work needs to be done, I just need these forms to send an email upon submission. Will provide a list of the fields in a text file upon choosing a programmer.
