MultiPage Form

I could do this myself, but really am too swamped to mess with it, so.....I have two paper forms that I use in day to day business. I need each one to be converted into a multi-page form on my unix server.

I will provide BOTH forms in a PDF format for you to work off of. I will also supply the html template for you to put the forms in. Please keep in mind that the form pages must stay in the main directory and utilize SSI for the header and footer (you dont have to do any of that - the website is already setup using SSI, which is why I want the forms to be integrated into the pages as is.)

You will need to create both forms (multi-pages!), provide and install the script, and test to make sure it all works.
