T-shirt design family crest

We have a crude mock up of a family crest that we would like to have created. The finished artwork would need to be usable by us to upload to a t-shirt printer like http://www.customink.com. We would also like to have the final project in an illustrator or PSD file in the event we need to make additional changes.
This project would be a success when the final design as been approved by my family and a print ready image is produced and uploaded to the t-shirt printing company. I would expect the image to be re-sizable without any noticeable degradation to the image.
The design is very simple, and will include a shield, banners with latin phrases, stars and some design freedom to make the overall finished product look professional. The mockup was designed by my son.
Family Name is "GRAUL"
Latin Phrase 1 is "In hac familia communicamus"
Latin Phrase 2 is "Nos defensionern infirmorum"
The shield field color should be grey
There are 6 stars the first being bigger than the rest of the stars.
In the image the feature that is not important or necessary to include on the shield is the orange and blue box at the bottom.
