Icon fixes

I have an existing ico file (attached). See the screenshot for existing colors depths and problem.

The icon needs some two fixes:

1. The icon needs the following color additions (and still look as good as in 256 colors):

48x48 - 16 colors

32x32 - 16 colors

16x16 - 16 colors

2. In different kinds of resolutions and color variations you can see "white" dots when viewing it in black background. See attached screenshot. You need to remove this for the following resolutions and color depth:

48x48 - all color depths

32x32 - all color depths
