I am looking for a freelancer to create a Microsoft form/access or excell for our company's vacation/PTO request process. We have 16 centers with a total of 50 employees.
the form should inlcude the name of person requesting the leave from drop down option, and also 16 centers (leave it as center 1-16 and I can reaname it)

it should include question or figour out of If the request is <30 days in advance for less than 5 days of contionus days of PTO or <60 days for > 5 days of PTO, then explain why requesting the PTO and approval is based on the AMD discretion

Expected PTO balance at start of PTO date request (numbers)
Adequate center coverage avalible confimred by the CA (y or N)
Name of the provider covering urgent Jellybeans (from the 50 provders drop down list)

Format/Design:
- I am open to suggestions for the format and design of the form.

Functionalities:
- The form should collect and store data for analysis purposes.

Ideal Skills and Experience:
- Proficiency in Microsoft Forms
- Experience in creating forms for data collection and analysis.

Are you open to cloud-based application for business? MS Form is handy tool but it has many limitation (query PTO, ...). I would suggest using Power Platform.
