Project Details:
- The client requires 9 posts to be designed for a carousel for both Instagram and Facebook:

Outline for IG + FB Social Media Posts for Freelancer to Design
Slide 1: Main Cover / Overview
- Design a cover featuring the title: "A Bridge to Care – At Our Rockwood Primary Care Home" (2023 Impact Report: Read about our accomplishments).
- Briefly introduce the impact report and encourage followers to swipe for more information.
- "Swipe to learn more about how Wallace made a difference in 2023."

Slide 2: Patient Experiences
- "How we helped 90% of our patients report good or excellent experiences at Wallace."
- Use visuals, such as happy patient photos or icons representing positive experiences.

Slide 3: Serving Vibrant Communities
- "Serving vibrant and diverse communities in East Multnomah County: Rockwood neighborhood where SE Portland and Gresham meet."
- Include a map or images of the Rockwood neighborhood to highlight the community served.

Slide 4: Challenges Faced by Our Community
- High rates of poverty, housing and food insecurity, and linguistic/cultural isolation.
- Info Stat: 41% of the population is low-income (under 200% of the Federal Poverty Level)
- Nearly all our patients have experienced bias based on class, race, gender, citizenship status, language, religion

Slide 5: Integrated Care Model
- Each medical patient has a care team that includes a provider, nurse, medical assistant, behavioral health, and patient care coordinator.
- New patients connect with enrollment assistance when they schedule their first appointment, to address that need before starting care.
- At the first visit we screen for social needs, explain the range of services, and offer support in removing any barriers in the way.

- Feature Thimbleberry Collaborative Farm and Milk Crate Kitchen on Wallace’s first “Farm-to-Table” project.
- Fact/Performance Metric: Each week, freshly cooked, ready-to-heat meals made using farm-fresh produce were delivered to 20 Wallace patient households,

- Fact/Performance Metric: Wallace collaborated with the city of Gresham’s Sam-Barlow School District to launch a new student health program / clinic at the Sam-Barlow High School. The “Big Bus” visits Gresham and Sam Barlow high schools on alternating weeks to offer students medical and behavioral care in a safe, confidential space, without needing to miss school and with no out-of-pocket costs.

- In 2023, Wallace Clinic focused on transforming jobs into careers for its workers in the community health field.
- Five staff members were promoted to manager-level positions within the organization, demonstrating a commitment to internal growth.
- Progress was made in establishing "career ladders" for all staff positions, with examples such as staff transitioning from reception/scheduling to medical assistant roles.
- The clinic provided opportunities for staff joining the dental team as sterile technicians to receive on-the-job training and become dental assistants.
- Collaboration with nurse practitioner training programs led to hosting externs in the clinic, resulting in a new permanent care provider for the team.

    Freelancer.com is a total scam. They scammed nearly 1572 USD of my earnings. Which I earn by providing my services on the site. My account was verified, and I completed each of my projects with 100% satisfaction. But when I made a payment withdrawal request, they closed my account and didn't pay my earnings.

    My humble request to all is to avoid the site. They don't pay freelancers who work here. If you still don't believe me, then see the attachment.
