3 Step Form | PSD customization

We are looking for someone skilled in slicing a PSD template we have into a workable 3 step email form. We would like you to take this psd: [login to view URL] and fully customize it as such: Step 1: Customer info Step 2: Services selection- place where our visitors can select what service they are interested in. Step 3: Review and verify info that the visitor just typed into the form. When they click the submit button, the data collected from all three steps is emailed to us. This should be quite the easy project for the right person and shouldn't take more than 3-5 hours.
