Order form for photos

I need an order form made for posting on my website. I have attached a word document with the information from my website on the prices of the different photos and packages available. I would like the customer to be able to choose the Package that they want so....Basic, Standard, Deluxe or Ultimate. Then the order form would automatically give them the proper number of Units for that package. It would also allow them to order additional poses, or units and know the correct cost of the additional units and poses. (as they change depending on the package choosen) The order form must be simple and easy to follow Please look at the attached word document. I need to incorporate all the items on this.... print size and number desired print package desired additional units additional poses if any digital images are wanted image numbers of photos ordered total price Please send example of your work to show that you can do this project. I will provide contact information and a logo for the order form.
