Convert Access 2010 Form to HTML 5 / CSS 3 (part 2) - repost

Simple little project. Simple screen layouts. No graphics, no code. Just HTML 5 layouts using CSS 3.
I need a five (5) simple MS Access form to be converted to HTML 5 / CSS.
I only require the web page design. No code is needed for database access.
Deliverables will be an HTML 5 page and a CSS 3 stylesheet.
