We are looking for an outsourced sales force, you must be able to provide your own prospective list for the National United States.
We are targeting:
PHYSICAL THERAPISTS
CHIROPRACTORS,
PODIATRISTS,
PEDIATRICIANS,
DERMATOLOGISTS,
ALLERGY SPECIALISTS,
GENERAL PRACTIONERS,
AMBULANCE SERVICES…
AND ANYONE WHO FILES AN INSURANCE CLAIM USING THE HCFA-1500 FORM (HEALTH CARE FINANCING ADMINISTRATION FORM)
What we are looking for is a long term solution to our need for a sales force. Payment scheme is setup as
$15 per client that signs up (will be paid at end of month 2)
plus $5 x 6 month = $45 total commission paid out per 'active' customer that you get to sign up.
** Active customer is defined as any customer that submits a minimum of 100 claims per month. (as our client base grows, this can be renegotiated with higher payouts)
Payments as previously stated will be paid to outside sales force company at end of month 2. Reasoning behind this will become apparent once we have selected the right candidate/company to support us.

To clarify, the $5 a month x 6 months is a calculation per customer obtained, so you would get an additional $5 a month for that customer for 6 months, as long as the customer is an 'active' 'participating' customer, active being active and still participating for the 6 months, any customer who cancels before the 6 months is no longer counted in the final months payout.
Participating means they are actively submitting the minimum of 100 claims thru our online service monthly.
If a customer cancels after 90 days, you will still receive the initial $15 + 5 + 5 = $30 for that customer. If the customer signs up and only processes claims (100 minimum) for the first month only, you will receive the $15 for the customer, no more residual income from that.
We are attaching an excel spreadsheet so that you can visualize the amount that you will make in the 6 months.

Funding: Escrow Only - No Paypal, No Funds Wired.
Funds will not be provided until that time we see the project in motion and with results.
Start Date: ASAP, must have real world experience, startup companies without experience need not apply.
