I need a lean, browser-based tele-reporting application that lets me, a solo radiologist, move through X-ray, CT and MRI studies at top speed. Every design decision should serve a fast reporting workflow: one login, a study opens instantly in an in-browser DICOM viewer, and I am already typing (or dictating) into a structured template with normal-finding presets pre-filled. Core viewer behaviour • Upload single or bulk DICOM studies, then auto-sort them by patient, date and modality. • Web viewer must deliver window/level, zoom, distance and angle measurements, cine scrolling and smooth multi-series comparison—no desktop install, just HTML5/WebGL. • Cached, encrypted cloud storage should feel PACS-like yet stay light on ongoing costs. Structured reporting • Templates that change automatically with modality. • Normal-finding presets are essential so that many common sections appear complete with a click. • Voice dictation inside the editor (I will supply my own microphone) with optional low-cost AI used solely to tidy grammar and punctuation—absolutely no diagnostic suggestions. • Keyboard shortcuts may come later, but please build with extensibility in mind. • Final reports generate branded PDFs that carry my logo and a digital signature. Workflow and security • Case statuses—uploaded, in-progress, finalised—must be trackable at a glance, with filters and free-text search. • Role-based logins for future technologists or junior readers, plus audit logs that record every access. • AES-256 encrypted storage, TLS in transit and adherence to Indian health-data norms (DISHA/HIPAA equivalent) are mandatory. • Architecture should support scaling to multiple radiologists without rewriting the core. Technology is your choice—React or Vue on the front end, Python, Node or Go on the back, perhaps Orthanc or DCM4CHEE behind an S3-compatible bucket—as long as startup time is near-instant and costs stay low. Document each component clearly and containerise so I can deploy on AWS, Azure or an Indian cloud later. I will sign off when: 1. A full DICOM study uploads, opens and scrolls smoothly in the browser. 2. I can complete a chest X-ray report with normal presets and voice dictation in under two minutes. 3. The PDF generated matches my sample layout and validates its digital signature. 4. Logs show my login, study access and report finalisation with timestamps.
