I need a report for the attached database that calculates commissions for each agent for each insurance company. The report needs to have the following format (example):
Period: 2012-05-01 -- 2012-05-31
Henrik John Drew Total
Insurance Company 1 $100.00 $50.00 $0.00 $150.00
Insurance Company 2 $75.00 $75.00 $25.00 $175.00
Insurance Company 3 $250.00 $0.00 $0.00 $250.00
Total $425.00 $125.00 $25.00 $575.00
The report is to be run for a specified date range based on deposit date.
I also need someone to redesign the forms. They work, but are pretty ugly. I like less white space and nicely aligned layouts. I would also like buttons for new contact, new carrier, and new policy on the commission statement form for easy access.
