This project needs to integrate with my current internal website "Companyweb" (IIS 7.0 - SBS 2008)
I am looking for a form to place on the front page of my current internal website to automate workflow
The form must contain the following
Line 1 --> user name --> should auto populate from %username%
Line 2 --> Client Name <--current client name for bypass
Line 2 -->%username% to Input Client phone number <-- must have and be restricted to 10 numbers only - no spaces or punctuation
Line 3 --> text box asking for a detailed reason for bypass <-- limit to 100 characters
Line 4 --> classification <-- a drop down arrow with three possible selections that I will provide.
Line 5 --> A button or some other interactive to initiate .CSV creation, email and finally to erase the form
All data to be out-putted to a .CSV file in their own columns and that .CSV file emailed to a specific user.
