These workflows fail when status is not a single source of truth. The app shows “Approved” but the flow is still running, or a retry creates duplicate approval records. In projects like this, the main issue is state management. Best practice is: one Requests list, one Audit log, and the flow updates status using a strict state machine with idempotent keys.
