Simple web based database for generating 3 simple reports

Need simple web database application for creating 3 reports in records of animals. Can be in dotnet or PHP.
The database is of dog sterilization program conducted by our organization for the dogs.
The data will be from a simple flat database of 8-9 colums.
The fields are
Case No
Catching Date
Dog ID
Dog Gender
Operation Date
Release Date
Address [GPS] co-ordinates
procedure
Catching Photo
Surgery Photo
The reports required are...
1. Daily Catching Sheet
2. Surgery Case Paper
3. Surgery Register
Further all images will need to be named as per the case number and their exif date changed to the dates of as per the database. the GPS data from database needs to be added to the Exif data and further, the address and GPS co-ordinates need to be written on the photos. [as per the samples]
