Script to Update/Insert data from json file to Mysql database

I have a Json formatted data file. I want to capture all data from file and save it in tables in mysql database.
I need the following
1)Create database and tables in mysql (see excel file attached..)
2)Script php/html (index file) to connect to database and onclick if records exists update else insert data
