The project is a series of automated questionnaires to create a Touchless Digital Health Declaration for specific retail environments.
I have an existing QR code system I pay for; if we end up moving forward, I will set you up with the appropriate access to complete work if required.
So currently, I have a system where I assign businesses a QR code, and they get a secure cloud-based folder that all of the questionnaires report. The problem is that they have only two forms available, and I want to have a custom version for different industries and regions.
We will design the form, and all yes admit any no do not admit. The pre-selected questionnaire requires a yes or no to a CTA response button.
The sequence of events in the process is.
1. Scan QR Code with a mobile device - phone
2. Complete questionnaire
3. Receive a response; based on the answer, receive an admit or deny access message back onto the mobile device
