This form should be completed prior to the appraisal interview. It is intended to help the Employee to decide what they want to discuss during the meeting, and will act as a reminder to ensure that all the points are covered.
The Employee should type their responses where indicated (Appraisee’s Response). Please ensure that enough information is provided to allow a full discussion on progress through the past year, and future objectives. One sentence responses are acceptable but single word answers, e.g. “Yes” or “No”, are not.
During the Appraisal Meeting the Line Manager will add comments to the form based on notes taken by during the Appraisal Meeting.
Once agreed and signed by the Employee, the appraisal form results will be mailed to HR and become a permanent record within the Employee’s personnel file.
A list of line manager will be provided to be added as a dropdown for appraisees and managers to view the departments forms or submit a new one.
