Create a script that will help create an email template

Looking for someone to create a questionnaire that will help complete an article. The system will ask me a series of questions and based on my answers, will produce results.
Also, some pages will ask unique questions and the answers will be populated throughout the articles.

Notice how this question adds an image which will be hosted on my server.
Here are some key points:
1) All of the snippets will be pre-written. No writing on your part will be required.
2) The content will need to be easily copied and pasted into a Gmail email, including inline images.
3) The format is important with headers, paragraphs and proper spacing.
4) This will be proprietary software, so no user accounts will be needed and the design isn't that important. It also doesn't need to be responsive in any way. The only concern is that it can be seamlessly pasted inside of a gmail email.
There should be around 12 - 15 questions similar in nature. At the end, we will have a beautiful article, ready to be added to gmail.
