MYSQL with Browser Interface

I need someone who can create a browser database query as follows:
log-in for the people who can
1. update a query
2. search a query
3. mutate a query
its a national register for accidents so people have to get information
and also input information.
it should also look modern and the search should be ajax-based.
i know this is not much to develop but i need professional advice/work. I would pay after finishing the project every month a certain fee for maintaining the database (protecting). its a longlife project.
